- When typesetting a page collection, in particular a trail page, we want to put the trail page itself before the table of contents, as a summary or introductory page. This page is not included in the contents list.

- Wrap the trail page, if it’s included, in a new <summary> tag. This appears to be a better option than inserting a “toc goes here” label. It will also make it easier to enrich the page layout of major sections; for example, a big section number, no page header, bigger top margin on that page.
The summary needs a <heading> and perhaps should include the url.
