- Include headings in the structure of published trail pages, such as PmWiki.Documentation Index. If present, treat them as section heads in the body of the document. Treat them as subsections of the summary page.

- The summary page name and any headings need to appear as nested pdf bookmarks, but be omitted from the contents page and not be numbered. The headings become sections in the document and all list items drop one level in the hierarchy. Change the dtd so that a <summary> can contain one or more <subsection> tags.

- Headings and nested lists are correctly handled. The dtd now allows subsections inside summary tags. It doesn’t appear to be a simple matter to generate pdf bookmarks while omitting these items from the table of contents.
