Improve print pdf format
We have made a number of minor layout improvements:
- shorten line length to improve the readability of pages, reports and books
- improve the report table of contents so major sections stand out
- improve the layout of book cover pages
There are also a couple of bug fixes:
- error in the DTD file for size attribute of style element (issue 00102)
- error in the tbents.txt file (issue 00103)
