Another round of patches and minor enhancements

• investigate use of <blockquote> markup for short quotations, including use of pull quotes — see Examples.Quotations
• demonstrate alternate forms of insert and delete markup — see Cookbook.Revision Marks
• fix strange problem when text with a background colour contains quote marks
• fix problem introduced when catching duplicate anchor names
• improve handling of vertical space after lists, so the following paragraph no longer indents
