Wikibook server version 1.0.11 released

This is a maintenance release to enhance 2 column output:

improve handling of images that span both columns
improve handling of margin notes — notes are flush right in left hand columns, flush left in right hand columns
make the columns slightly wider

There are a couple of other minor improvements:

allow % as a hyphenation character in a long URL
support cover=left and cover=right for the article cover option added in version 1.0.9
fix a L^AT_EX emergency stop if headings include highlighted text
change the default highlight colour

This release also removes the tarball of the original tbook-1.5.2 software from the distribution, making the download about half the size of earlier releases. It can still be downloaded from Install the Server.
